The nomination said...
My daughter has a very rare chromosomal disorder combined with severe hearing and dementia disabilities. She is 40 and is now in end of life care but living at home with me.
Last year her health took a very bad turn for the worse. She had a big operation in February to remove an infected and burst appendix which also involved some bowel removal. Since then her whole health has been a struggle for her.
Dr Bellhouse, my daughter’s Psychiatrist asked Jerry to step in as a liaison nurse. Jerry has been amazing. He has got to know not just my daughter but her siblings and myself. He has attended very complex meetings (CHC funding) on a number of occasions and really was a knowledgeable advocate for her.
Jerry carries a heavy work load but always rings to find out how things are going. Jerry has visited my daughter and me when she was an inpatient and expected to die. He has been so supportive of her and the whole family throughout very difficult times.
He still continues to support my daughter.
