Public Choice Awards 2018/19

Where you tell us what care and compassion in mental health means to you

If you have received outstanding care and support from a member of our staff then this could be the perfect opportunity to give them the warmest possible pat on the back and nominate them for an award.

We are asking you to nominate the person or team that you feel made all the difference to the care you or someone close to you has received.

Perhaps you are part of another NHS or social care organisation and you work alongside our staff. If you have seen examples of excellent and compassionate care and practice then you can nominate too.

Sometimes it’s the smallest things that matter most, especially when people are at their most vulnerable.

It could be the person for whom nothing is too much effort; or maybe it is the staff member who always takes time to explain what is going on and listens to your views.

It maybe a person who – with words or actions – helped someone when they felt like giving up; or it might be a person whose regular support has enabled you to make positive changes in your life.

Maybe it’s the team at the end of the phone that gives you time to talk when you’re concerned about a loved one in our care. The staff member who smiles and says ‘hello’ when you visit a ward and remembers your name; or the person who always turns up on time for a home visit.

Please take this opportunity to make a huge difference to those staff that have made a big difference to you and send us your nomination today!

How to nominate

If you have had experience of our services from February 2017 onwards you can nominate a staff member or team, clinical or non-clinical, by completing our nomination form, telling us your story and the reasons for your choice.

To nominate someone you need to complete the official Nomination Form. You can download a form from this website. You can then complete the form electronically and email to us at, or you can print the form, fill in by hand and send to us using the Freepost address.

If you are unable to download the form, you can request a hardcopy version by emailing us at or calling 01603 421120.

The Public Choice Outstanding Care and Compassion Award is divided into two sections:
  • Children, Families and Young People (CFYP) services
For example: Youth and Family teams, Child and Adolescent Mental Health Services, children’s learning disabilities, Wellbeing services (up to 24 years), and Perinatal and Infant Mental Health Services.
  • Adult services
For example: Adult community, adult inpatient services, older people’s services, secure services, learning disabilities, and Wellbeing service (25 years and over).

You can use the same form to nominate to either of these categories – just tick the relevant box on the form.

To be in with the best chance of winning, your nomination must explain how your experience of care or contact with this person made a real difference and why, through their care and compassion, they helped to make a change for the better in your life or in the life of someone close to you.

Please fully describe why they stand out for you and offer any examples you can.
